Resources to assist in developing/writing liability shields are currently located on one of the 4-H Youth Development websites. While these are specific to 4-H Youth Development programs, they can be easily adapted for a variety of audiences. Photo Release Forms

The use of photo release forms is applicable to all Extension employees who are intending to use individual and group photos for promotion and other marketing activities. Accident Reporting

Ohio State University Extension has established guidelines and protocol for reporting accidents. Information may also be found on working with the media during a crisis situation. The materials, found on the following website, also include the appropriate form to be completed when there is an accident.
