Data Collection Instruments for Local Use

Most data collection instruments are available to the educator/programmer in a text format.  The files may be saved to your local computer, therefore, providing the user the opportunity customize the instruments by editing names, dates and adding specific local touches. Don't' forget to "Save As" and select  the "save as text" box, highlighting Text (*txt).   This will save time in utilizing a text file instead of saving and converting an HTML file.

  1. Authors of the instruments have provided approval for use of their materials if source is given credit.
  2. Evaluators must use the instruments at their own risk and discretion.
  3. E-mail links are provided to send authors input, feedback and a note of thanks if you find them helpful.
Links will be provided for several "view only" files (in pdf format) to demonstrate a professional look with the use of various software.

Needs Assessments
These data collection instruments were used in order to identify
specific needs of program areas, communities or issues identification.  

Follow-up Survey
The follow-up survey technique was used to explore the impact of
participation in a workshop or request after a length of time. Do the
participants apply, after a length of time, the skills and knowledge
gained by their participation in the intervention (workshop, request, etc.)?
