Data Collection Period

Data should be collected for all activities or products that occur from May 1, 2005 through September 29, 2005.

Collection and Submission of Data

Data information will be gathered using the attached Pilot Evaluation Program Worksheet.   One worksheet should be used for each product or activity.  The worksheet is on the GLCASH website for download, http://www.ag.ohio-state.edu/~agsafety/glc/index.html

The deadline for data submission for the months of May, June and July is August 15, 2005.   Data collected for August and September will be due October 12, 2005.   The PI or her/his staff can arrange with GLCASH to have more frequent collections of data or to design a custom data collection plan.   Regardless the deadlines listed above are the minimum that must be achieved.

Entering Data

Data should be entered directly into the Word document, allowing for additional space as needed.  If you are unable to access the Word document, enter the information by hand and use continuation pages as needed for additional writing space.

Activity Type

If you are documenting an activity, select the box that best describes what activity has taken place.  If you are unsure of what to select, choose other and write or type in your own classification of the activity.

Activity Classification

Classify the activity as an evaluation, education, research, or intervention activity.

Product Only

Product Type

If you are documenting a product, select the box that best describes the product type.  If you are unsure of what to select, choose other and write or type in  your own classification of the product.

Product Title

List the Title of your product

Journal/Publication Name, Author(s) and Language(s)

If the product is a type of publication, list the name of the Journal/Publication it will be published by, the names of all authors, and any language it will be published in other than English.

Activity/Product Target Group & Population Description

•   Describe the targeted group for your activity or publication and list how many indivuduals

     were targeted and from what state. 

•   Select a special population groupo.  You may choose more than one.

•   Select the appropriate ethnicity of your target population, if applicable, otherwise select N/A.

Collaborators

List any groups or individuals that collaborated on the activity or product.  In addition, list any outside funds or services received that enhanced the scope or reach of the activity or product.
