Permissions Request Form

This form is for requesting access to edit a County/Center/Region website. These sites are edited using existing email accounts set up on the college email server. These email accounts are those that end with @cfaes.osu.edu. If you do not know what your account is, you can contact your Regional Tech to find out.


Requester Information (i.e. the person filling out this form):

Request Account Permissions for:

Name: Account: (without the @cfaes.osu.edu)
User Role: Editor Reviewer Administrator* (limit 2 administrators per site)
Site:

* There is a limit of 2 Administrators per site, however most content administration can be done by a user with both the editor and reviewer roles.

If you would like to request permissions for another site for the same user, select "Submit and Autofill" to submit the above request and have this form appear again with all the information above already filled in. Otherwise, select "Submit" to submit the above request and this form will appear with only the requester information input.
