Evaluation Instrument
Change for Healthier Living People (Project Cambiar)
is a diabetes education program aimed at assisting Hispanic seniors to understand diabetes better and make necessary dietary and lifestyle changes. The program was the first culturally sensitive diabetes education program in Cleveland Ohio. The evaluation instruments asked participants to respond to food frequency questions, health care access, self-image wellness, knowledge and attitudes about diabetes, demographic characteristics and medical and physical data. The program was a joint effort of the Diabetes Association of Greater Cleveland, the Hispanic Senior Center, Ohio State University Extension, Cuyahoga County, Neighborhood Family Practice, and the Spanish-American Committee.
