Listed below are the forms necessary for advisors and members to enroll for the 2004 4-H year.

The New Advisors page has the necessary forms for individuals seeking an advisor position.

If you are a member, please do not fill out and send in a youth enrollment form, as they must all be turned in to a club organizational advisor for their records.

All forms listed below are available from the extension office if you do not have access to a printer. Please do not request copies from the extension office unless it is absolutely necessary, as we are trying to reduce costs.
